Zlín Clinic was fantastic. Clean and very professional nurses. Super efficient. I was 49 and received anonymous donor eggs, and my kids are the most fabulously adorable kids (according to others’ reports as well as mine lol). The city is somewhat quiet (make sure you bring some good books to read!) I went with an agency, but I think you could communicate directly with the clinic with no problem.
